John Dorian, J.D. to friends, is a boyishly handsome, self-deprecatingly funny, likeable guy who would be confident if he only realized his assets. Four years of medical school have prepared J.D. for his first day as an intern at Sacred Heart Hospitalbut he has his doubts. As J.D. is thrown into the chaotic world of hospital life, we not only get to see the quirky characters that he encounters, but also hear the humorous commentary of thoughts streaming through J.D.'s head. By J.D.'s side is his best friend since college, the handsome and confident Chris Turk. As a new surgical intern, Turk struts around with a gregarious and cocky demeanor that hides a terror that he only admits to J.D. Both guys initially have the hots for their attractive fellow intern, Elliot Reid, but J.D. quickly finds out her beauty is only surpassed by her hyper-competitive attitude. Guiding the interns education are those with more medical experiencewhich at this time is pretty much everyone. Dr. Robert Kelso, Chief of Medicine, wants the interns to think they can count on himuntil they actually need something. On the other side of the personality spectrum is Dr. Perry Cox, a steamroller of a man short on patience. However, beneath his gruff presence hides a truly good guy that J.D. learns to respect. Watching over the interns with a caustically teasing but caring demeanor, is Nurse Carla Espinoza, a woman who is motherly and sexy at the same timewhich appeals mightily to Turk.
J.D., Turk, and Elliot are now two weeks into their hospital intern program, and things are going well: the procedures are becoming easier, and no one's life has been endangered yet. Meanwhile, J.D. and Turk's apartment has become a safe haven away from the stresses and challenges at the hospital, and Elliot has become a frequent visitor. Although J.D. denies it, Turk thinks his buddy is interested in being more than just colleagues with Elliot. Meanwhile, Turk has his own eye on another member of the hospital staff: Carla. At the hospital, J.D., supervised by Dr. Cox, is assigned to Will Forte, a 31-year-old patient admitted with a serious case of pneumonia, and possibly cancer. It turns out that the patient doesn't have cancer, but when J.D. tries to warn him about the possible effects of his 3-pack-a-day smoking habit might have in the future, his advice falls on deaf ears. Elliot is having her own difficulties with Carla after complaining to Dr. Kelso that Carla neglected to run a test on a patient. The rest of the nurses side with Carla, and Elliot's fumbling attempt to apologize gets her deeper in trouble. And Turk's numerous attempts to woo Carla fail, until he gives an impassioned speech that convinces her to go on a date with him.
Elliot asks J.D. for a kiss, but after they're interrupted by Dr. Cox J.D. has to make a move in 48 hours before going to "the friend zone". Meanwhile, Elliot has problems with Dr. Kelso calling her sweetheart and goes for Dr. Cox for advice, who tells her to confront Dr. Kelso. She does, but later realizes her mistake and thanks Dr. Cox for his advice on fighting her own battles, when Dr. Cox really wanted was to be left alone for once. And J.D. feels neglected by Turk, who has been spending a lot of time with Carla and so little time with him. J.D. even thinks of reporting what could be a mistake from Turk on the very first man he closed up after surgery. J.D. finally remembers that his 48 hours are almost over and rushes to Elliot, but he's too late. He's in the "friends zone".
The three interns are each assigned patients with various illnesses. Statistically speaking, one of the three will die. J.D.'s patient is 86-year-old Mrs. Tanner, who is in renal failure; twenty-year-old David Morrison is Turk's hernia patient; and Elliot's patient Mrs. Guerrero, who speaks only Spanish, might have lupus. After diagnosis, the three doctors try to bond with their patients. When no one is watching, Mrs. Tanner leaves her room and goes to the park with her family for a birthday party/picnic for her five-year-old granddaughter. Pursuing J.D., after having some s'mores and cake, brings her back to the hospital, where she tells him she is now ready to die. Turk and David take a moment to bond by watching a classic football game on television; Elliot has Carla teach her some phrases in Spanish so she can communicate with Mrs. Guerrero. All three patients develop life-ending complications. J.D., Turk, and Elliot are shocked by their first professional experience with death; and in their own ways each accepts that death happens, and life goes on.
J.D. points out what seems to be a technical mistake to Dr. Kelso, but it was really an attempt of Dr. Cox to save a woman without insurance using a dead guy's insurance. Dr. Cox is at first mad at J.D., who is asked by Dr. Kelso to join him in a golf round. Dr. Kelso and Dr. Cox show J.D. their different ways of looking at medicine (Kelso's medicine-is-a-business look and Cox's patients-first look), and Cox is suspended for using one patient's insurance on another. Meanwhile, the janitor intimidates J.D., Elliot discovers the healing powers of her boobs and Turk gives Carla a pen he took from the "ass box" (the box with the stuff taken out of patients butts) after a small misunderstanding.
Dr. Cox is still facing the threat of suspension, and coincidentally, J.D. is assigned to a special patient: Jordan Sullivan, an important board member. She is very demanding and J.D. ends up sleeping with her after he confronts her. Only he didn't knew she is Dr. Cox's ex-wife, and now the only one who can save J.D.'s mentor from being fired by the evil Dr. Kelso. Meanwhile, Elliot is assigned to take care of a shrink whose jaw has been wired shut, and for whom she ends up becoming a patient. Carla's mom makes a scandal after Turk spends the night at Carla's, and when she breaks her leg Carla blames Turk for it.
J.D. has finally earned the spot as brightest student, but he's got competition: Nick Murdoch, who seems to be nicer, friendlier and has a better butt than him. But not even J.D. can hate the guy, for he is so nice. But he is surprised when he finds Nick sitting in a corner crying because he can't take the pressure of medical internship. Meanwhile, Turk is having problems at the operation table and doesn't tell Carla, who keeps bothering Dr. Cox about it. And Dr. Cox gives Turk a very unique suggestion on how to calm down before surgery.
J.D. and Turk become heroes after saving a cameraman's life on TV while heading to a strip club. But Turk quickly becomes the target of a new publicity campaign led by Dr. Kelso, exploiting the hospital's diversity. Meanwhile, J.D. worries about his evaluation. Elliot is going out with Carla, but in the last minute she blows her off and later Elliot run into her in a bar. Carla feels bad about herself and apologizes to Elliot.
J.D. gets to see the hospital through the eyes of a patient when he's admitted for appendicitis. Elliot examines him and he tells everyone how cold and rough she is, which drives her crazy. She tries new ways of warming her relationship with the patients, but none of them work. J.D. doesn't want Turk to operate him, and that hurts Turk's feelings a bit but he's the one who does J.D. anyway. Meanwhile, Dr. Cox is trying to show off to his mentor, Dr. Benson, the ex-chief of medicine, who is a patient at the hospital. The only thing in his way is Dr. Kelso, who almost dismissed the patient 1 hour before he almost died. At the end, they all celebrate Thanksgiving at the bar, each with their own accomplishments and failures.
Extremely annoyed with a patient that won't shut up and with Elliot, who won't discharge her, Dr. Cox releases his anger on J.D., and Carla gives Dr. Cox a piece of her mind for messing with her intern. Dr. Cox ridicules J.D. in front of the whole hospital, and J.D. decides to take action and asks Carla not to protect him anymore. She is deeply offended and J.D. feels very bad for losing such a good friend. But despite all that, he's still Bambi (even though the Janitor thinks "Scooter" is a better nickname). Meanwhile, Turk bravely fights for his right to eat lunch on the bench outside the hospital after Dr. Kelso threats him saying that's his bench.
It's Christmas, and even though J.D. and the others don't really feel like celebrating, Turk makes a huge deal about it (specially after one of J.D.'s patients suddenly returns from coma). But his faith and happiness are gone after a very busy night at the hospital, where several people are brought in for the most diverse accidents. Turk wonders if there really is a God, and if so, why he didn't protect all those people. His misbelieve in religion changes after he is guided to find a patient of Elliot's, who ran away from the hospital at the edge of delivering a baby. Speaking of babies, Dr. Cox has J.D. videotape the birth of the child of one of his friends, but J.D. messes it up because he didn't change the tape and thus the birth was not shot.
A woman slips and falls on the wet floor of the hospital. Afraid she might sue the hospital (and consequently having some big heads roll), Dr. Kelso asks J.D. to stand by her and be friendly so she won't sue them. But J.D. is very busy with Dr. Cox's 24-hour marathon to keep his ICU patients alive. Dr. Kelso's assignment fits like a glove for J.D. to help Elliot, who is desperate for some attention from Dr. Cox. J.D.'s relationship with the injured lady deepens and even though he hasn't seen her face (she got stuck in the ultrasound machine), he thinks about asking her out, but is afraid she might not be attractive. After advising Elliot to face her fears, J.D. decides to do the same thing and asks the lady out. Meanwhile, Turk snaps at Carla when she steals fries from his plate, but after a few arguments he tells her he loves her.
Alex is about to emerge from the M.R.I. machine in front of an apprehensive J.D., who has just asked her out but doesn't know what she looks like. She's hot. The two of them have a romantic dinner at a restaurant one night and are enjoying a second date the next night when they are interrupted by a page from Dr. Cox. Later in the week, J.D. and Alex arrange to get together at the same restaurant. Their plans are messed up again when J.D. gets caught up with a patient while Alex waits patiently for him at the restaurant. He tries to make it up to her the next day, but because of his busy and unpredictable schedule, she decides to break it off. He makes one more attempt to get back together, but she turns him down. After thinking about it overnight, Alex reconsiders her decision. She surprises J.D. with a picnic basket dinner that they share in an empty hospital room. Meanwhile, Carla and Turk are enjoying their newly announced love for one another. But their usually wonderful lovemaking session isn't so wonderful when Turk can't perform. Since J.D. is a little preoccupied, Turk turns to his fellow surgical intern Todd for solace. And after much begging and pleading from Elliot, Carla confides in her: the problem wasn't Turk, it was her inability to reach orgasm. (Elliot shyly confesses she has never had one, and Carla gives her some surprising laundry advice.) A day later, Turk and Carla discuss their relationship and decide that even though they've said "I love you" to each other, it doesn't mean that they have to plan the wedding tomorrow. Turning to hospital business, Dr. Cox fills in for Dr. Kelso on morning rounds. The interns are surprised to find that mean as he is, Dr. Cox is much nicer to them when they can't give him instant right answers. Dr. Kelso is aghast to discover that everyone in the hospital seems to have lost their fear of him, and demands to resume his tyrannical rounds routine.
J.D. chooses Alex over Elliot when the two argue over missing medication, based solely on his chances of (quoting Turk) "booty"; totally ignoring Elliot's feelings. Elliot is very hurt, and won't even speak to him. Later, J.D. finds the pills on Alex's purse and they break up. He apologizes to Elliot, who confesses she was jealous of him while they rush to bed. Meanwhile, Carla gets a ride with Dr. Kelso and for a brief moment starts to believe he may actually have a caring heart. But Dr. Kelso proves her wrong after she hides behind his car avoiding being seen with him by the other nurses. And Turk develops a conflict with Dr. Cox after J.D. finds out Dr. Cox has secret feelings for Carla.
After going to bed and spending an entire day having sex, J.D. and Elliot go to work trying to hide their new secret romance, but everybody already knows it. Soon the relationship deepens, and Elliot start feeling jealous of what Turk and Carla have together. J.D.'s silly jokes and remarks don't help either, and after a heart breaking discussion about J.D. not respecting her, they decide to end whatever they had.
The awkwardness between J.D. and Elliot is so big that they can't even be in the same room together. Seeing his friends drift apart, Turk picks Elliot to work with him on a project, hoping it will bring the two together again. Todd, in a desperate attempt to hit on Elliot, tells Elliot that Turk picked her as his geek partner so she would do most of the work. Elliot abandons Turk, who reveals his real intentions. Finally, the awkwardness between J.D. and Elliot starts to cool off and they start having fun again like old times (throwing watermelons from the top of the building, for example). Meanwhile, Dr. Cox releases his anger by destroying an entire lab and later drags J.D. to a bar (with J.D. in his pajamas), where he opens his heart to him. And Carla confronts Dr. Kelso after he denies one of Carla's patient coma whishes, while the hospital lawyer develops a weird crush on her.
J.D., Elliot and Turk are receiving their first medical students, and now they finally get a chance to see what it feels like to be on the other side. Elliot can't wait to unleash all her medical student frustration on her intern. She starts off tough with her rude and annoying student, but softens after learning he is the son of the CEO of the corporation that owns the hospital. It takes Dr. Kelso to open her eyes and show her that she doesn't have to kiss his ass for Elliot to get tough again. Turk's student is a very attractive young woman, for whom Dr. Cox is immediately attracted to. But he doesn't know his way around women (specially when he's getting over Carla), and needs Turk to give him a little help. And J.D.'s student carries a lot of resemblances to J.D. when he first started. J.D. doesn't know how to handle him, and tired of cleaning up after him, J.D. tells Josh that he shouldn't be a doctor. Josh quits the program, and J.D. has to pull a Dr. Cox-esque persona to encourage him through negative reinforcement.
J.D. has a patient who's rude to him and the nurses, and eventually dies from cancer. J.D. feels awful for not giving him his full attention and worries maybe he could have saved the guy if he was nicer to him, even though that's not true. Meanwhile, Elliot is convinced that Dr. Kelso is the one singing a song called "Tuscaloosa Heart" in a tape given to her by a patient who claims he went to college with Bob. And Dr. Cox finds himself trapped into a love square: he's falling in love with Kristen Murphy, his intern, just as his ex-wife reappears demanding sex and he can't avoid her and there's also his crush on Carla, that made him stand Kristen up. When she finds out about the other women, she leaves him, and Perry is once again at mercy of his ex-wife Jordan.
An assortment of moms and dads descend on their offspring at home and in the workplace. J.D.'s father is a gregarious yet lonely divorcé who's desperately trying to be just one of the guys. Elliot's folks, on the other hand, are a snooty couple whose pompous behavior dismays their daughter. Similarly rattled is Turk, unnerved by the bonding of his girlfriend, Carla, and his brassy mother. But the most vexatious parent by far is the janitor's dad.
Turk' competitiveness is starting to annoy J.D., specially when it crosses the line of personal jokes and becomes a patients matters (whether or not a patient should go into surgery is their latest feud). Meanwhile, Dr. Cox finally has a taste of good coffee, but thanks to his feud with Dr. Kelso, the only nurse who was able to make it gets fired. It takes a public apology from Dr. Cox for Dr. Kelso go back on his decision and bring back "coffee nurse". And Elliot meets a very attractive patient whom she instantly identifies, but has a really hard time getting him to ask her out.
J.D. fears being contaminated with Hepatitis B. after a needle accidentally sticks his forearm. From then on, he tries to avoid any contagious patient, trading one of them with Dr. Cox. When the results are out and J.D. is clean, he tries to make up for his foolishness by spending some time with the contagious patient out of his own free will. Meanwhile, Elliot has a hard time combining her new relationship and keeping up with studies and chooses her studies over him. And Turk obsesses about getting in shape after Carla makes a fat joke about him.
Jordan's brother Ben is admitted at Sacred Heart for having an accident with a nail gun. Ben and Dr. Cox are long time buddies, and they go out for drinks after Ben is released. They bring J.D. with them, who quickly bonds with easygoing, spontaneous picture taker Ben. But while playing pool, Ben's hand starts to bleed, which takes them back to the hospital for more tests. When the results are back, J.D. learns that Ben has leukemia, but won't believe it since both Elliot and Turk had wrong results on their patients. Unfortunately, J.D. isn't so lucky and has to face the sad reality of telling Ben his condition. To Be Continued
Ben takes the news of his diagnosis pretty well - better than his sister Jordan and J.D. at least. Remarkably, it is Dr. Cox who is the voice of reason and compassion, until his own fears get in the way. Meanwhile, Turk and Elliot have their own issues to deal with. Turk is upset because he wasn't chosen to assist on a difficult surgery and Elliot is criticized during rounds by Dr. Kelso for the 100th time.
It's J.D.'s last day as an intern. It's been quite a year for the three new doctors. But nevertheless they have work to do. J.D. passes on one of his patients that keeps returning to the hospital to Elliot, who had just gotten the task of running a physical on Jordan. Elliot passes the patient to Turk, who refuses to take it. When the three decide to rock-paper-scissors the patient to see who gets it, they realize what they're doing and decide to make up for it by teaming together to give the patient a surgery so he won't have to come back again. In order to do that, they have to get Dr. Cox's support, Dr. Kelso's approval and a board member to approve it - Jordan. Everything turns out right for them and the patient gets his surgery. When they're finally celebrating both their first day as residents and the success of their teamwork, Jordan shows up. She decided to take Perry's advice to "shake things up a little" and reveals all the secrets she knows: Perry's feelings towards Carla and Turk's awareness of it; Dr. Kelso's lies about giving Perry a promotion; Elliot's true feelings for J.D.; and the cherry on the top of the sundae: the time she slept with J.D. "Enjoy your summer" she says.
Relationships between the staff of Sacred Heart Hospital have been shaken since Jordan's revelations. Turk is sleeping on J.D.'s bed after Carla kicked him out of his own. Elliot is having a hard time facing J.D. everyday at work. J.D. desperately tries to get Dr. Cox's attention back, unsuccessfully. Eventually they all work it out: Elliot and J.D. start facing each other again, Carla finally gets over her issues with Turk and Dr. Cox starts to torment J.D. again, meaning it's all back to normal. Meanwhile, the Janitor keeps haunting J.D. across the hospital.
J.D., Elliot and Turk find themselves alone in the hospital night shift and face for the first time responsibilities without supervision. Luckily, they have Carla there to support and guide them through the night. Meanwhile, Dr. Cox doesn't know what to do when he realizes he has feelings for his ex-wife Jordan, and is asked (actually, forced) to introduce Dr. Kelso at a social event where he will be receiving an award from the medical board.
Dr. Kelso is sponsoring one the medical students with a trip to a medical conference in Reno if they present an interesting case, which causes a rush of all sorts of cases to become "interesting". J.D. is not into the trip as much as the other students and tags with Dr. Cox on treating one of the hospital's money contributors, but when her case ends up being one of a kind J.D. reveals his true intentions and wins the trip, disappointing Dr. Cox. He explains that his main goal is not to please Dr. Kelso, but to become a good doctor, even better than Dr. Cox. Meanwhile, Turk is also in a rush to get a new medical appliance on Dr. Kelso's giveaway day that only occurs once a year after his marriage anniversary when he shows up in the hospital in a good mood. Unfortunately, he's late and misses his meeting with Kelso, but gets what he wants anyway after finding out the truth behind Kelso's giveaway day. And Elliot is still trying to befriends with Carla, despite Carla's reaffirmations that things are okay between them. When they both treat an elderly patient who's getting breasts implants, they both reflect on what they'd change on themselves.
J.D. insists that Carla should open up to him, share a little bit more about herself. When she finally starts to do that, J.D. screws it up by telling Turk he thinks she's crazy. Now he has to work his way to regain her trust. Meanwhile, Elliot is so desperate for attention from Dr. Cox and that she even agrees on giving the dying news to the patients families, which causes her great deal of emotional stress. Finally, Dr. Cox, feeling sorry for her, tells her they'll never be colleagues while he's her superior. And Turk sees himself in an ethical dilemma when Dr. Kelso picks him over a female surgeon to spend a weekend in Mexico. He realizes the female doctor should've won it, and the only reason she didn't is because she's a woman.
Elliot sleeps with a handsome doctor she hardly knew, and soon becomes the hospital tramp a title she's at first embarrassed to have, but quickly sees the good side of it when all women who work there start coming to her for sex advice. Meanwhile, Turk suffers from his back after operating with Dr. Amato, a very short surgeon, and learns that he can be a good doctor without being a part of the surgical "boys club" after he hurts Elliot's feelings by talking about her in a trashy way to the other surgeons. And J.D. finds out that wearing a white coat doesn't make him a better doctor when he mistakenly causes a patient to lose his sense of smell. Upset by having Dr. Cox throw in his face that he made another mistake, J.D. proves Dr. Cox he didn't cause any harm to the patient and drops the coat. And Dr. Kelso is just dying for an opportunity to lash out his anger on Dr. Cox.
J.D.'s brother is in town. He came all the way from their hometown to pick up a car and drive it back for 300 dollars. His older brother is fun and easygoing, and quickly lays his charms on Elliot. J.D. tries to prevent anything from happening between them, and realizes that it's only because he's ashamed of his brother, who was the the man' in high school and now is just a bartender living with his mother. When J.D. confronts him, he leaves. Meanwhile, Turk is thrown out of a patient's funeral for not knowing his name and feels even worse because earlier he made a bet with Dr. Cox over another patient's life. Dr. Cox later tells him that doctors make jokes and don't attend funerals because they can't get attached to the patients or else they won't stand the pressure.
All the male doctors is euphoric with the visit of Julie Keaton, the pharmaceutical sales rep, except for Dr. Cox, who still believes in treating patients for their best health instead of trying to get some attention from the hot Ms. Keaton. When she finally offers herself to go out with him, he declines. Meanwhile, J.D. and Elliot argue over what to do with one of Elliot's patients. He thinks she should wait and see, while she thinks the patient should go up to surgery. The patient dies, but Dr. Kelso says sending her to surgery was the best thing to do since she had no chances of survival whatsoever. And Turk tries to talk Carla into entering a practitioner nurse course so she won't be so underestimated by the doctor's staff, but Carla chooses to spend more time with her boyfriend instead of wondering what could have been in a few years.
J.D. and Turk are going through some financial problems and in an effort to save some money they start to steal stuff from the hospital like toilet paper and pudding. The janitor becomes suspicious of J.D., but ends up taking home a computer himself. J.D. also gets mad at Turk for stealing a hundred bucks from him for their free clinic work, but after he realizes that Turk and Carla are going to get married he lets it go. Meanwhile, Elliot has some financial problems of her own when her father cuts her loose of all the money after she took a stand and refused to take on OBGYN. And Dr. Cox has to choose between his hot girlfriend, Julie, and his old fling, Jordan, who shows up pregnant at his apartment.
Ever since her father cut her off financially, Elliot's been feeling the weight of the world on her back. While having to find a new affordable place to live, she also has to face the threat of being sued by a patient whom she told had only 8 months to live (and the problem is 9 months have passed by ever since). Carla and Jordan hit a sore spot when Carla gives her opinion on Jordan and Perry's relationship. Meanwhile, J.D. has a fight with Dr. Cox and the two dispute over who gets to give the better treatment to two patients with the same prognosis, after J.D. gets so full of himself for picking up a totally uncommon flush eating bacteria on one of Dr. Cox's patients. J.D.'s patient ends up dying, though he didn't do anything wrong. All he didn't have was luck.
Elliot still doesn't have a place to live. She experiments crashing at the hospital for one night, only to be fully reprehended by Dr. Kelso. She finally takes up on J.D.'s offer after she realizes that living on a truck at the hospital parking lot isn't exactly the best accommodations she can get. Meanwhile, J.D. think he's lost his mojo when he can't even kiss the giftshop girl when they go out on a date. First he blames it on the hospital and it's disgusting cases, but later at his place he proves himself wrong when he and Elliot sleep together. Turk and Carla are having romance problems because she feels he's not being as generous to her as she is to him. And Dr. Cox freaks out when he lets Jordan move in to his place due to her pregnant status.
Renewed sex with J.D. calms Elliot, who's facing personal and professional dilemmas: She's broke, and patients are bailing out on her.
Comeuppances are in store for Cox, Kelso and J.D. from, respectively, a hypochondriac, a surgical patient and Elliot, who wants more than just sex from J.D.
J.D. bonds with a patient who's waiting for a heart transplant and discuss with her their idea of what death must be like. J.D. is also touched with the cycle of life that surrounds the hospital - a place where people live and die all the time when he has to ask a husband to choose between his pregnant wife or his unborn child. Meanwhile, Elliot puts her foot down and demands a feminine locker room to Dr. Kelso, since she's fed up with having to chance in front of all the men. Dr. Kelso is reluctant at first, but has to concede Elliot's demands after she discovers some legal issues on it. And Turk decides it's time to propose to Carla, but a fat kid swallows the engagement ring which doesn't stop him from popping the question anyway. The big surprise is Carla's answer she needs to think about it.
Turk is still waiting on Carla to answer his proposal, and asks his brother to stay over a few days to help him deal with it. But Kevin Turk, the image of happy marriage, comes with bad news: his wife has left him, and now Turk has to put aside his feelings and deal with his brother. Meanwhile, Dr. Kelso is faced with having to let go Dr. Townshend, an old friend of his and one of the nicest doctors around when he finds out Dr. Townshend doesn't have the energy to update his medical techniques. And Elliot accidentally finds out the sex of Jordan's baby, becoming the object of Dr. Cox's anxiety.
Elliot meets a nice guy at the bar, but when she finds out he's a male nurse (a "murse") at Sacred Heart she has doubts about getting involved with him. Meanwhile, Turk keeps asking Carla to marry him, but she still won't give an answer. Tired of trying to find out why, Turk decides to stop asking, which causes Carla to reflect on why she won't say yes. After a deep introspection, she calls Turk and asks him to ask the question right before his doubts turned into actions as he was about to go out for coffee with a waitress. And J.D. feels like he has not stories to tell today; after all it's all going great for him for the first time since he started working at the hospital. Therefore, we see what Dr. Cox feels like and the pressure he puts on himself everyday. His psychiatrist, his subordination to Dr. Kelso (whose life he saved after refusing to sign for a physical he never gave) and his paternal feelings for J.D., who got a pad in the back for the very first time.
J.D. and Turk think they've got bad karma after hitting a car and causing an accident while playing golf on the hospital roof. While Turk wants to keep it a secret, J.D. thinks things will get better after they come clean with a patient who was injured on a car accident caused by a flying golf ball. Meanwhile, Dr. Cox bends over backwards to give Jordan as much comfort as possible on her labor day, and she confesses to J.D. that Dr. Cox is the real father. And Elliot seeks advice with Carla on how to tell Paul she's crazy, because they're getting more intimate and she can't hide it anymore. But Carla is feeling too crazy herself with the upcoming wedding.
While J.D. struggles with whether to tell Dr. Cox the truth about the baby's paternity, he gains some valuable insight into Dr. Cox's troubled mind and his relationship with Jordan when he meets Dr. Fisher, a former student of Dr. Cox. When Dr. Kelso prevents Turk from playing his pre-surgery basketball game, Turk retaliates by cutting the cord to Kelso's beloved Ms. Pac Man machine. Meanwhile, Carla worries that she's lost her sex appeal since she got engaged, and Elliot's lucky streak continues as she makes a new friend in the janitor.
J.D. flirts with the wife of a patient who's been in a coma for the past two years, whom they all call T.C.W. (Tasty Coma Wife). He feels guilty for doing that, even though she wants to go ahead with it. Meanwhile, he can't stand listening to any more relationship problems, as everybody dumps their issues on his ears: Elliot sabotaging her relationship with Paul over beef jerky; Carla finding out her engagement ring passed through a patient's digestive system and Dr. Cox's unsucessful attempts to get some attention from Jordan, who can only think about her baby right now.
Turk takes J.D. up to surgery, and right from the start J.D. steps all over Turk when trying not to be the geek among the surgeons. That hurts Turk's feelings, and J.D. feels bad for not thinking about his long time friendship with Turk when he made the jokes. They finally come to terms at the end, when Turk decides to get over this because it was just another one of J.D.'s goofs, as many he has made in years. Meanwhile, Paul hears Elliot say "I love U2" while she was rearranging her cd's and thinks she said "I love you too". That takes him to a whole different level, while Elliot is actually trying to find a way to break up with him. And Dr. Cox decides to get even with Dr. Kelso by telling everyone he's dead, revealing what people really think of him.
JD struggles to juggle work while dating a recent widow, Cox babysits his daughter, and Turk is haunted by erotic dreams.
J.D. carries on his romance with the Tasty Coma Widow, but finds himself on a dead end after realizing she's a drama queen and the romance is dead when there's no drama. Meanwhile, Dr. Kelso makes Dr. Cox give a class on how to approach the patients nicely, in order to prevent possible law suits from the patients due to harsh treatment. Carla's mother dies and she rushed to get married. Turk stops her from running over her dreams in order to fulfill a momentary emptiness caused by her mother's death.
J.D. and Turk's college buddy Spence drops by for a visit, and accidentally tells Dr. Cox that Jordan's baby is his. At first freaked out, Dr. Cox soon faces up his responsibility and stands up for his role as a father. Meanwhile, too much partying with successful Spence causes Turk and J.D. to show up late and drunk for work, which Dr. Kelso fully reprehends them for. They compare their lives with Spence's and wonder if being doctors is really the right thing for them. Elliot's feeling low since she can't measure up to Dr. Kelso's standards and the Janitor starts another feud this time over paychecks with J.D.
A run in with her old flame Sean and an accident in her new car leads Elliot to the realization that nothing good has happened to her in the three years that she has been at Sacred Heart, driving her to take drastic measures, including a complete makeover, both inside and out in an attempt to change her luck. Meanwhile, JD gets stuck with a patient whose diagnosis he can't quite pin down and when Dr. Cox denies him any assistance, he looks to Carla and Turk to help him out. While Dr. Cox is busy not helping J.D., he takes the time to make amends with Dr. Kelso.
J.D.'s feeling left out on his friendship with Turk and gets hurt when Turk brings The Todd to the bar where they were supposed to catch up on their friendship. While talking to one of his patients who's gay, Turk admits to himself that he's scared of opening up to another guy. Meanwhile, Carla is driven crazy over an unidentified urine sample and threatens the janitor, while Elliot tries to balance Sean and her career out so one doesn't end up getting on the other's way.
Finding difficulties on training his interns, J.D. calls out to Elliot for some help. She indicates Sean, who gave Elliot great tips based on his dolphin training that fit perfectly for her interns. This only makes J.D. even more jealous of Elliot, but Sean is OK with it since he doesn't consider J.D. a threat. Meanwhile, Dr. Cox finds the ideal pediatrician for his son: Dr. Norris, who acts and looks just like him (besides the hand puppet thing). And Cox is quick to find a conflict with him once Jack develops a cough that freaks out both him and Jordan.
Sean is a bit surprised and disturbed to learn about Elliot and JD's history together, but it turns out his anger is really a cover up for the fact that his "little" work trip is actually six months long and he doesn't want Elliot to explode. J.D., who has been watching their relationship develop, finally acknowledges that he still has feelings for Elliot and that he has to come to either accept her new relationship or do something about it. Meanwhile, Dr. Cox. is forced to swallow his pride and actually ask for help when he decides to throw his hat in the ring for the Residency Director position at the hospital. Carla picks up an extra shift as a surgical nurse, which allows her to spend the day working alongside with Turk, to his great annoyance. (NBC)

A chance meeting in the elevator sparks a new friendship between J.D. and the mysterious Danni whose ongoing dialogue about her ex forces J.D. to talk about his feelings for Elliot. Meanwhile, Carla's brother Mark comes to visit, but he still hasn't forgiven Turk for mistaking him for a waiter at their mother's funeral. Unfortunately, Turk can't even apologize because Mark refuses to speak anything but Spanish. At the hospital, Dr. Cox relishes his new job promotion but is surprised when J.D. doesn't follow his lead in contradicting Dr. Kelso's orders. (NBC)

Turk is happy that he finally makes it through a solo surgery without the assistence of Carla. Meanwhile, Dr. Cox dreads the fact that he is getting older as his back goes out. JD also makes a discovery as he finds out that many of the hospital staff don't know the Janitor JD does. This leads to a surprising revelation of how the Janitor really feels about JD.
Elliot's feeling uncomfortable using clinical terms for her patient's private parts a result of a repressed childhood and an accidental orgasm she inflicted on a patient during a pelvic exam - so Carla tries to teach her how to say "vagina" and "penis" without blushing. Meanwhile, Dr. Cox crashes at J.D. and Turk's after taking bad advice from J.D. Carla suggests no more sex until the wedding and Turk only comes to terms with it after learning the real meaning of romance from a patient with prostate cancer. Ultimately Carla backs down on her own idea when she has a stressful day at work finally letting people make fun of her.

She goes to seek Dr. Kevin Casey's advice but becomes frustrated when she learns that the epiphany toilet, which the Janitor secretly installed on the roof, is getting more attention than she is. Meanwhile, Turk asks JD to be his best man at his wedding, which sends JD into sheer bliss momentarily until he hears a phone message that makes him question his friendship with Turk.
Jordan's brother and sister return but Dr. Cox learns his brother-in-law Ben hasn't visited a doctor about his cancer in the 2 years he has been gone.
Jordan's friends Allison and Maddie come to town to provide moral support for Jordan after her brother's death however, their constant presence does not provide any relief to Dr. Cox. Meanwhile, Turk tries to impress the new female surgeon, Dr. Miller, but his first encounter serves only to leave him baffled by the opposite sex yet again. Attempting to reinforce the policy on sexual harassment at Sacred Heart, Carla is forced to teach a course where Dr. Kelso is the number-one student.
An ever present butterfly prompts the question on what could have happened different as Turk looks for his lucky doo-rag for surgery, and Elliot tries to find a lost stuffed animal for a child needing a bone marrow transplant.
JD must keep a secret when Carla's "what-if" guy asks her out on a date. Meanwhile, on the other side of the argument, Carla hounds Turk for checking out other women. Elliot is also on the spot as Dr. Cox and Dr. Kelso both advise her not to give her patient drugs as they suspect he is an addict faking pain.
Turk has to confront a young patient of his when he accidentally cuts a nerve and the boy (who happens to be a concert pianist) loses use of one of his hands. Meanwhile, one of Elliot's patients unintentionally teaches her a lesson about her relationship with J.D.
JD hooks up with Danni again after Sean returns, but he can't forget about his passionate night with Elliot. Meanwhile, Perry realizes he might actually have a crush on the one doctor he loathes.
Elliot asks Sean to move in with her, which only adds to JD's problems. Turk is unsure why Dr. Miller's attitude towards him has changed, until he finds out Carla un-invited her from their wedding. Kelso outrages Dr. Cox when he announces that Sacred Heart will begin doing full body scans, starting with an ever present hypocondriac patient who wants any freebies he can get.
JD is unsure what to do when realizes he made a mistake telling Elliot he loved her. The Janitor squares off against Dr. Cox after he loses the election of the janitors to Randall. Carla's brother Marc constantly puts Turk in a tough situation when Turk forgets to write his wedding vows.
The big day for Turk and Carla has finally arrived but a patient holds up Turk. With Elliot still angry at him, JD attempts to reconcile her with Sean as a way to make every thing right again in their friendship.
The new Dr. Clock arrives with her spaced out personality and doesn't cease to call J.D. "Johnny". But J.D. is fine with it, because there is nothing that is going to bring down his last week of being a resident before he officially becomes a full fledged doctor.
Clock tries to reconsile JD and Eliott, while waiting to see which one Cox will be given the Chief Resident position. Meanwhile, Dr. Cox, Turk and Janitor are forced to work together to help a patient.
It's trouble for everyone at Sacred Heart. Dr. Cox is having issues with Dr. Clock consulting his patients as well as the fact that he and Jordan are technically still married due to some filing problems. J.D. has issues of his own as he can't shake the tag of Co-Chief Resident (while Ellito keeps the Chief Resident tag). Elsewhere, Turk begins to doubt his abilities after letting a patient die, and isn't helped when he finds out J.D. has no confidence in him either.
J.D. is shocked to hear Dr. Cox (who he has called in for a pep talk) tell his interns that eventually, they will screw up and kill a patient. Though upset at first, Dr. Cox's words get to J.D. as well as he fears his first time killing someone will soon be upon him. Elliot is upset with Molly after she disagrees about a patient's promise to attend drug rehab. Carla loses Rowdy and she and the Janitor go looking for him.
Elliot becomes unsure she wants Molly as her mentor when she finds out that Molly doesn't have a perfect home life. J.D. is tested in leadership skills when Dr. Cox yells at him in front of his interns. Carla and Turk try to come to an agreement when Turk continuously wakes Carla up every night.
When Dan resurfaces, J.D. is shocked to learn his father has passed away. The reactions from his co-workers help J.D. a little, but the one person, Dr. Cox, who he wanted to be there for him is too busy with J.D.'s patients. Meanwhile, Dan sits drunk in the bath tub the whole day and Turk finds out he has Type II Diabetes.
When J.D. finds Dan with Elliot, Turk tells him to use the hurt brother excuse to get them to break up, just like Turk uses his diabetes to get stuff from Carla. But then J.D. realizes he wasn't hurt at all... Dr. Cox and Dr. Kelso unite to try and torment their common enemy -- Dr. Clock.
When Molly takes a job in another city, she and J.D. end up having a night of kissing and he tries to rush to sleep with her before she leaves. Dr. Cox goes on Ambulance duty with a maniac who he eventually finds out is a woman who has lost her son to an accident.
J.D. is approached by a malpractice attorney, asking him to do a risky procedure on her father. Meanwhile, Elliot guides Doug to a new career path and Turk struggles to avoid a hypochondriac patient.
With the case against Turk on his mind, J.D. attempts to end it with Neena, but she won't take no for an answer and keeps him wrapped around her finger until J.D. summons Jordan to get involved. And Elliot sets out to prove she isn't the doctor she was 4 years ago and doesn't need to rely on anyone anymore... and that scares Dr. Cox.
J.D. takes matters into his own hands, when searching for a relative for his favorite patient. Meanwhile, Elliot is fed-up with co-workers not taking her seriously, and discovers she can get her way by flaunting her femininity with some help from Carla and Jordan.

Where Elliot's cool is almost lost is when she realizes The Janitor may have had more in mind for their cafeteria "coffee date" when he shows up in suit and tie. His explanation for his attire leads to an a cappella sing-off between his "group" and Ted's band. Meanwhile, Turk starts taking his diabetes more seriously when he realizes the dire consequences of the disease, until Carla reminds him that his sense of humor is what helps them both cope.

While planning his birthday, J.D. finds a list of things he wanted to accomplish before he turned 30. He enters a triathlon to begin. Elliot, discovers a side of her boyfriend Jake that freaks her out. Turk is working hard to impress his bosses and takes on a patient who wants to be hypnotized during surgery rather than using anesthesia, all in effort to be promoted.
J.D. is asked to introduce Dr. Kelso at an awards dinner, so he wants to find out something good about him. Dr. Kelso and Dr. Cox each have a patient whom they feel deserves the benefit of a new experimental treatment, but there is only one open slot. Carla and Turk try to help Elliot get her old job at Sacred Heart back. The Janitor taunts J.D. with the made-up game of Jiggly Ball.
Dr. Cox's religious sister Paige comes to visit for her nephew's baptism. Turk is finding out just how unsexy baby making can be. Elliot helps him realize that Carla finds the whole process to be romantic. J.D. and the Janitor start a budding friendship.
J.D. mistakes an elderly woman's happiness as her wish to stop treatment for a fatal illness. Turk and Elliot struggle to find the cause of a patients body pain and Carla tries to organize the annual group photo.
In a "Wizard of Oz" homage, J.D. is trying to go home, Turk searches for a heart donor and Carla worries that she doesn't have the courage to become a parent. Meanwhile, Eliot doesn't believe she has the brains to lead a question and answer session. The Janitor is is painting color-coded lines throughout the hospital for easier navigation.
J.D., Turk, Elliot and Carla are interrogated about the mysterious death of a patient. Dr. Kelso learns that J.D. and Turk were away tracking down a patient, Elliot was traumatized over finding out that her make-out buddy was married and Carla was busy recruiting hospital personnel for a group lottery purchase.
J.D. meets the klutzy Julie, and when he becomes smitten, Elliot attempts to help out with romance advice; Carla tries to find Cox's soft side when he refuses to kiss his son; Turk and Janitor put together an air band to win a contest, but Kelso gives them a tremendous obstacle.
J.D. is annoyed that Julie doesn't laugh, so Elliot encourages J.D. to tell her his feelings and Turk tries to use psychology to fix the issue. Meanwhile, Carla is upset when Janitor refers to a new nurse as a "hot young Carla," which intensifies her growing concern that she is too old to conceive.
J.D. is still dealing with his recent break-up and trying not to kill intern Keith, who has become annoyingly perfect. To make matters worse a favorite patient is in a serious coma and each of the staff finds their own way to deal with it. Dr. Cox and the Janitor bond over drinks at the bar and Carla makes it her mission to make the hospital's gym female friendly. Meanwhile Turk suggests that J.D. and Elliot find themselves 'booty calls.'
Dr. Kelso is outraged when he discovers that a bird has been living in Sacred Heart. He orders the Janitor to dispose of it immediately. However, the Janitor likes the bird so much he secretly keeps it as a pet. J.D., annoyed with Keith, tries to get his intern fired but all doesn't turn out as intended.
J.D. and Dr. Cox go through the five stages of grief with their favorite patient who is declining in health. Carla forces Turk to go on a string of double dates, including Eliot and her booty call Keith. Dr. Kelso hits Ted with his car, thus Ted finally has the upper hand with a potential lawsuit.
J.D. is sick of perfect intern Keith and he's not the only one. Elliot's relationship with Keith is revealed so the other interns think he is getting special attention. Dr. Cox boasts about being named "The Best Doctor in the City" by a local magazine, thus Dr. Kelso assigns him to the most difficult patient in the hospital to deflate his ego. Turk and Carla continue to struggle in their quest for conception.
J.D. urges Cox and his fellow doctors to do special, unconventional things for their patients, but J.D. himself is not sure he can shave his head for a woman undergoing chemo after meeting a woman who loves his hair. Turk and Carla attempt to eliminate all the stress in their lives to better their chances of conceiving, but Turk has to compete for a surgical resident spot and Carla loses Dr. Kelso's VIP patient. Elliot can't stop giving new boyfriend Keith stupid gifts.
Turk finds out that Carla is pregnant before she does, so J.D. convinces Turk to tell the entire hospital staff to surprise Carla with the news. But Carla wants to personally share her baby news with everybody so Turk and J.D. have to quickly contain the hospital gossip. J.D. also tricks the Janitor into swallowing a tiny GPS tracking device. Meanwhile, when Elliott puts Dr. Cox into an embarrassing situation, he decides to get even with Elliott through Keith and Carla.
When J.D. rearranges his schedule to allow himself more time to spend with Turk, Turk secretly resents hanging out with J.D. in that time because it interferes with his own "Turk Time." Meanwhile, Cox and Jordan go on a double date with Elliot and Keith and Carla covers for Kelso at work as he grieves for his dead dog. Later, a patient almost dies after giving up part of his liver for his brother, and the hospital staff learns about making sacrifices for the people they really care about.
J.D.'s brother Dan returns for a surprise visit and Elliot learns that J.D. has been telling Dan that she still likes him. Carla and Turk freak out when they begin to discuss baby names. Dr. Kelso assigns Dr. Cox to assist Ted with malpractice claim paperwork after Dr. Cox insults Dr. Kelso's son. Meanwhile, a patient with weight-related problems needs an MRI, but he's too big for the machine.
Turk and Carla turn J.D.'s old bedroom into a nursery for the baby. The Janitor seizes an opportunity to shove J.D. out of the picture for a day. Elliot uses her mother to learn information about Keith's peers. Turk accuses Dr. Cox of not being hip, and Perry accuses him of the very same thing.
J.D. asks Dr. Cox for lunch but runs into annoying former patient Jill Tracy, who unexpectedly teaches him something about responsibility. Elliot and Carla discover that Todd's been lying about all his hospital "conquests" and suspect that he's been less than truthful about other issues. Turk warns them not to meddle in his affairs.
Dr. Cox starts drinking after his decisions result in the death of three of his patients. The staff tries to cover for him at the hospital while everyone tries to convince him how inspiring he is for them. Turk starts his new rotation with orthopedic surgery, but isn't so fond of his attending doctor's touchy-feely methods. Thus, J.D. and the Janitor attempt to help Turk find ways to bond with his new supervisor.
J.D. feels that the same things happen over and over again at the hospital. Dr. Cox returns to work, but Elliot believes that he has lost his usual confidence. Elliot calls Dr. Cox on his behavior. Carla gets fed up with Turk that he can do a lot of stuff but she can't since she is now pregnant.
J.D. challenges a urologist's assessment on a consult for a patient in need of a risky surgical procedure. He also starts to fall for her. Elliot considers breaking up with Keith for being too weak. Dr. Cox picking on him does not help his case.
J.D. plans a date with Dr. Kim Briggs on their night off, but Elliot gets in their way. Perry and Jordan celebrate Jack's milestone. The hospital pick a name for Turk and Carla's baby while somebody else is eagerly awaiting the results of a pregnancy test.
